You get you soul essence gem from one of the buildings to the right of the keep's entrance (as in, you leave the keep and go right).
The Soul Essence Gem is an item that is used to hold part of the Dragonborn's soul if the option to become partially soul-trapped is chosen at the end of the quest "Chasing Echoes." When found during the quest "Beyond Death," it will remove the Weakened Soul ability that is gained upon becoming soul trapped.
There are numerous luminescent cracks in the ground throughout the Cairn. Interacting with one will fill the largest empty soul gem on hand, although soul fissures will not fill the Black Star or Azura's Star.
The Soul Cairn is one of the most interesting locations in the entire game, and you will soon understand why after experiencing the realm in all its glory. With such a giant realm, there will always be something hidden around every corner in the Soul Cairn for you to discover and explore.
