No One Knows His Original Name Originally, Yamato was an unnamed orphan. He's never had a 'real' name, only a series of code names. While in Anbu, he was Tenzo, and then Yamato.
Yamato is the temporary leader of Kakashi Hatake's Team 7, consisting of Naruto Uzumaki, Sakura Haruno, and Sai. "Yamato" is actually a codename given to him for the purposes of joining Team 7, his real name being Tenzo. Yamato, however, seems to prefer going by his codename while near his teammates.
Short answer: Yamato is alive and was released from the trap he was in during the war. His current job is keeping constant watch over Orochimaru.
Because Tenzo is his Anbu code-name. His name before that was Kinoe when he was working for Danzo. Then he got a new code-name Tenzo when he went in the Anbu. Then, when he quit the Anbu to do missions and be a normal shinobi, he changed it to Yamato.

Danzō's right arm was implanted with at least ten stolen Sharingan. Danzo's stolen Mangekyo Sharingan from Shisui Uchiha. Because of the stolen Sharingan eyes and the cells from the First Hokage (which Tobi suspects that he implanted them all with Orochimaru's aid), Danzō was an immensely powerful and skilled ninja.
